Please fill out the following order form complete, with your name, address, city, state or province, zip code or postal code, email adress, and phone number.

In the order description box below: please state what you would like to order: ie 11 oz. mug, 15oz. mug, coaster, mouse pad, etc.  If you want 2 pictures on your mug please submit them both.  Let us know what text you would like written on your item.  Give us all the information you can, and we will make up a template and email this to you for your approval.  With this information we will be able to serve you better.
